Bishara began experimenting with electronic and experimental music after being influenced by classical music, and becoming interested in Tangerine Dream. After seeing the silent film The Cabinet of Dr. Caligari in 1920 and the 1922 horror classic Nosferatu, he became obsessed with horror film scores. He joined the industrial band Yesterday’s Tear, later known as Drown, in the late 1980s. By 1994, he had signed a record and touring deal with Drown. He also did a brief tour with Prong. Bishara’s film score career began in 1998 with the Biblical drama Joseph’s Gift, but the majority of his subsequent work has been in horror films, beginning with Unearthed and The Gravedancers in 2006 and Night of the Demons in 2009.

Bishara has worked in film production in addition to composing, providing sound design services for John Carpenter’s Ghosts of Mars, and serving as music producer for Repo! The Ghosts of Mars project entailed programming and remixing Carpenter’s music and determining how it would be integrated into the film. Bishara collaborated on the song “Shroud” on rapper Tech N9ne’s album Special Effects in 2015. In 2018, he collaborated with Tech N9ne on the Planet album, producing and co-writing the song “Brightfall.” The song inspired a series of horror shorts starring Tech N9ne and Bishara. Acting Bishara acts in several of the films he is musically involved in, usually portraying demons or other supernatural entities.

Bishara is known to enjoy horror and has stated that he prefers writing for the genre over others: “It was the one that gave me the most energy. There was always something more compelling about being on the verge of everything disintegrating.” He is known for his avant-garde style and one-of-a-kind approach to instrumentation and composition, which lends itself particularly well to horror films. Music critics have praised his disregard for horror film score conventions, citing “gut-wrenching string dissonance, haunting electronic sounds, and sudden crescendoes,” as well as his use of strings and percussion to create effects intended to elicit “maximum shock effect” in the listener.
