McMillan’s debut book, Mama, was published in 1987. McMillan was dissatisfied with her publisher’s minimal promotion of Mama, so she pushed her own debut novel by writing to hundreds of retailers, primarily African-American bookstores, and the book quickly sold out of its initial hardback printing of 5,000 copies. McMillan gained national notoriety in 1992 with her third novel, Waiting to Exhale. It was the second-largest paperback book transaction in publishing history. The book stayed on The New York Times bestseller list for several months, and by 1995, it had sold over three million copies. The novel helped to change Black popular cultural consciousness and bring a female Black middle-class identity to the forefront of popular culture.

McMillan was recognized with introducing the inner lives of Black women professionals in their thirties who are prosperous, single, accessible, and unhappy. Forest Whitaker directed the 1995 film adaptation of the novel, which starred Whitney Houston, Angela Bassett, Loretta Devine, and Lela Rochon. Another of McMillan’s novels, How Stella Got Her Groove Back, was made into a film in 1998, starring Angela Bassett and Taye Diggs. McMillan’s novel Disappearing Acts was adapted into a direct-to-cable film with the same name in 2000, starring Wesley Snipes and Sanaa Lathan and directed by Gina Prince-Bythewood. McMillan’s A Day Late and a Dollar Short premiered on Lifetime in 2014, starring Whoopi Goldberg and an ensemble cast that included Ving Rhames, Tichina Arnold, Mekhi Phifer, Anika Noni Rose, and Kimberly Elise. McMillan also wrote The Interruption of Everything (2006) and Getting to Happy (2010), the follow-up to Waiting to Exhale.
