McCarthy co-edited Kings of Bs: Working Within the Hollywood System with Charles Flynn, a book about the great B-movie directors that was published in 1975. He relocated to Los Angeles and worked as an assistant to Elaine May at Paramount Pictures from 1974 to 1975. He assisted her in the editing of Mikey and Nicky (1976). McCarthy served as the director of advertising and marketing for New World Pictures in Los Angeles from 1975 until 1977. He also worked as a film critic for The Hollywood Reporter in 1975, but was fired a year later. McCarthy later became the English-language edition of Le Film français’s manager in 1977. The next year, he was hired as a Hollywood editor for Film Comment.

McCarthy worked as a reporter and film reviewer for Daily Variety from 1979 to 1989. In 1990, McCarthy earned an Emmy Award for his work on the PBS documentary Preston Sturges: The Rise and Fall of an American Dreamer. Visions of Light (1992), Claudia Jennings (1995), Forever Hollywood (1999), and Man of Cinema: Pierre Rissient (2007) are his four cinematic documentaries. The National Society of Film Critics and the New York Film Critics Circle selected Visions of Light the Best Documentary of the Year. For more than a decade, Forever Hollywood has being shown at Grauman’s Egyptian Theatre.

He became the film review editor of Variety and Daily Variety. In his book, Howard Hawks: The Grey Fox of Hollywood, published in 2000, he writes about the producer/director Howard Hawks. He published Fast Women: The Legendary Ladies of Racing in 2007. McCarthy is also the author of Des Ovnis, Monstres, et Du Sexe: Le Cinéma Selon Roger Corman (2011). After leaving Variety, McCarthy began writing for IndieWire. In October 2010, he was rehired as The Hollywood Reporter’s main film reviewer, succeeding Janice Min. He authored the preface to cinematographer John Alton’s book Painting with Light, which was published in 2013. McCarthy was laid off from The Hollywood Reporter in April 2020. McCarthy then began writing frequently for Deadline Hollywood in late 2020.
