Gemmell has appeared in both stage and television dramas. She starred opposite Colin Firth in Fever Pitch, based on Nick Hornby’s novel of the same name, and had another leading role in the comedy/drama 2 January (2006). She appeared as Detective Constable Kerry Cox in the first series of the BBC police procedural Silent Witness, which aired in 1996. In 2006 and 2014, she returned to the series as a different character. She played the title character’s mother in Tracy Beaker’s Movie of Me in 2004. When Carly was three or four years old, she had a new boyfriend who was abusive to her, forcing her to live in a children’s home.

She first appeared in EastEnders as Debra Dean, the mother of a teenage girl who, like Tracy Beaker in Tracy Beaker’s Movie of Me, abandoned her child when she was an infant. She appeared as Rebecca Sands in two episodes of The Bill in August 2009. Gemmell has played two different characters in the BBC’s police drama Waking the Dead three times. Her first appearance was as DI Jess Worral, a former lover of DSI Boyd, in the episode Special Relationships in 2002. She next appeared as Linda Cummings, an exceptionally intelligent serial killer, in the seventh season episode Sins in 2008. Endgame, the fourth episode of the eighth season, saw Gemmell reprise his role as Cummings.

Cummings’ role reprise is a first in the show’s history. Gemmell appeared as Joanne in Episode 8 of Jimmy McGovern’s BBC drama Moving On. Gemmell portrayed Lady Shonagon in the BBC Radio 4 Woman’s Hour adaptation of Robert Forrest’s The Pillow Book. In early 2013, she played Jen, the wife of an adulterous civil servant, in the Channel 4 drama Utopia. Gemmell played Octavia Putney in five episodes of the television series Penny Dreadful in 2015. She began her role as Lady Violet Bridgerton in the Netflix series Bridgerton in 2020. Ruth Gemmell reprised her role as Carly Beaker for My Mum Tracy Beaker in 2021, when Tracy and Jess moved in with Sean, and Cam and Carly argued about Tracy.
