After failing to obtain a role on the television series Webster, McCullough auditioned for the role of Robin Scorpio, the 6-year-old daughter of Robert Scorpio (Tristan Rogers) and Anna Devane (director/actress Finola Hughes). The producers made her audition 12 times before giving her the part, which became a great success. McCullough made her acting debut in the fall of 1985 with the Asian Quarter plot, which featured her with veteran actor Keye Luke for the majority of the storyline. McCullough earned her first Daytime Emmy Award for “Outstanding Juvenile Female in a Drama Series” in 1989, when she was 11 years old.
She returned to the show in 1998, but later left to explore other acting chances, including co-starring in the primetime dramas Once and Again and Joan of Arcadia. She also wrote and directed the mockumentary Lil Star, which is about girls competing in childhood beauty pageants and draws on McCullough’s own experiences. In the 2001 film Legally Blonde, she played Amy, one of Elle Woods’ sorority sisters. Following brief returns to General Hospital in 2000 and 2004, she returned on a permanent basis in October 2005, with her character Robin Scorpio, a doctor who had graduated from medical school. McCullough revealed that she would be leaving General Hospital again to pursue a career as a director.

In 2018, she went off contract again, announcing her retirement from acting to pursue a career as a director.
