Alexander’s acting career began in the late 1980s. She made her television debut on the sketch comedy show FTV in 1985. Alexander has primarily worked in film and television since the early 1990s, with supporting roles in films such as CB4, Joshua Tree, What’s Love Got to Do with It, Poetic Justice, and Sugar Hill. Alexander was cast as Catherine Duke on the NBC comedy series NewsRadio in 1995. She remained on the show until season 4, episode 7, “Catherine Moves On,” before returning for a final appearance in the season 5 premiere episode, “Bill Moves On,” to honor Phil Hartman. In the medical drama series ER, she played the recurring character Jackie Robbins. Alexander has appeared as a guest on a number of other television shows.

Alexander received critical acclaim in 2000 for her portrayal of Fran Boyd, a drug-addicted mother, in the Emmy Award-winning HBO miniseries The Corner. She later appeared in the films Emmett’s Mark and Dark Blue, and she co-starred in the Lifetime television film Perfect Strangers with Rob Lowe. She played Alexx Woods, a medical examiner, in the CBS police drama CSI: Miami from 2002 to 2008. Alexander left CSI: Miami shortly before the 2007-2008 season ended. Her last episode aired on May 5, 2008. She returned to the role of Alexx Woods for a guest appearance in the episode “Smoke Gets In Your CSI’s” on February 2, 2009. She returned as Alexx Woods in the episodes as a guest star.

Alexander was cast as the lead in the HBO drama pilot Treme, which premiered on April 11, 2010. She portrayed a bar owner in a New Orleans neighborhood devastated by Hurricane Katrina. From 2010 to 2013, Alexander starred in David Simon’s award-winning HBO television series.  After four seasons, the show was cancelled. She was later cast as Maya Lewis, Kerry Washington’s character Olivia Pope’s mother, in Shonda Rhimes’ drama series Scandal. For her performance, she was nominated for a Primetime Emmy Award for Outstanding Guest Actress in a Drama Series in 2015. Alexander was cast as Queen Latifah’s title character’s older sister in the HBO film Bessie about iconic blues singer Bessie Smith in 2014.
