O’Keefe began her modeling career at the age of eight, when she did some work for a jeans company. O’Keefe dropped out of high school in the middle of her junior year to play Marguerite “Maggie” Cory on the soap opera Another World. She then landed a role on Nash Bridges as Cassidy, the title character’s daughter. Because she and her mother relocated to Hollywood, O’Keefe completed her education through the mail. O’Keefe made her feature film debut in 1998 in Halloween H20: 20 Years Later, and has since appeared in films such as The Crow: Salvation, Whatever It Takes, and Devil In The Flesh 2. Taylor Vaughan co-starred in the teen film She’s All That (1999) in 1999.

O’Keefe continued to film both films and Nash Bridges until the series ended in 2001. Out for Blood, in which she played a vampire named Layla Simmons, and Venice Underground were two of her later films. She has appeared in a number of television shows, including Dharma & Greg, Boston Legal, Two and a Half Men, Charmed, The Evidence, The Big Bang Theory, and Tru Calling. In 2007, O’Keefe was cast in a starring role on the television series Prison Break; she remained with the show through its third and fourth seasons. O’Keefe has stated that she “truly loved” playing the role of Gretchen Morgan on Prison Break because it “challenged” her and because Gretchen was the “polar opposite of her” and “was badass.”

In 2014, O’Keefe appeared in the film Merry ExMas and had a recurring role in VH1’s TV series Hit the Floor. The same year, she was cast in the recurring role of Jo in The Vampire Diaries’ sixth season on The CW. She played Jo again in the first season of Legacies. She made an appearance in the video game Command and Conquer Red Alert 3: Uprising. O’Keefe was photographed for the NOH8 campaign in June 2013. O’Keefe appeared in the video for 3 Doors Down’s “Let Me Go.” Queen George Clothing is O’Keefe’s own clothing line. O’Keefe debuted her own jewelry line, Q, in 2012. Together with her friend Victoria Pratt, O’Keefe is a licensed bounty hunter.
