In 2013, he appeared in several television shows, including Beauty and the Beast and Copper, where he appeared in three episodes. Gianniotti appeared as Lord Julien in the first season of Reign. He appeared in several episodes of Murdoch Mysteries (2013-2014) as Leslie Garland. He played Freddy in the now-canceled television series Selfie (2014), and he later appeared in the made-for-television drama film The Secret Life of Marilyn Monroe. Gianniotti played Andrew in Backpackers in 2015.
In 2015, he was cast as surgical intern Dr. Andrew DeLuca in the final two episodes of Grey’s Anatomy’s eleventh season. He was promoted to series regular on January 8, 2016. Gianniotti was one of the Top 25 Canadian Immigrant Awards presented by Canadian Immigrant Magazine in 2019. For his role on Grey’s Anatomy, Gianniotti won the Tell-Tale TV Award for Favorite Actor in a Network Drama Series in 2021.
